A written statement confirmed by oath or affirmation for the use of evidence in court. For example: "A former employee swore an affidavit relating to his claim for unfair dismissal.

$6 for the first acknowledgment, $1 for each additional signature, $6 for oaths, and $0.50 per 100 words of a deposition.
